Appointments
Our office is open to serve you from 8 a.m. to 5 p.m., Monday through Friday. To schedule an appointment, please call from 8 a.m. to 4:30 p.m. Our offices are located at:

If your physician’s office schedule is delayed, our staff will gladly offer to reschedule your appointment. If you find that you need to change your appointment, we would appreciate notification of our office at least 24 hours before your scheduled visit.
New Patients
If you are a new patient, please arrive 15 minutes early to complete the necessary patient information forms or download/print the following forms and complete before you arrive:
- Patient Information Form
- Health History Form
- HIPAA Form
- Patient Bill of Rights
- Insurance Authorization Form

It is requested that all new patients bring the following to their first appointment:
- Insurance Card
- Copies of pertinent diagnostic tests and medical records
- All current medications
- Completed history and physical form, if available
To ensure optimal care, please provide us with hospital records or results of GI testing at another facility. Please have copies of the reports sent to our office prior to your first visit.
Fees, Billing and Insurance
We participate in most insurance plans including Blue Cross/Blue Shield, Medicare, Medicaid and area HMOs and PPOs. As a service to you, we will file the appropriate reimbursement claim forms with your insurance carrier. You are responsible for payment of deductibles, co-payments and estimated insurance balances at time of your visit as well as any outstanding balances on your account. Payment will be accepted in cash, check or major credit cards. If you are uninsured or experiencing financial difficulties, please call our Business office at (850) 841-1166, so that we may assist you in establishing an agreeable payment schedule for your account.
Please be aware that many insurance plans require pre-authorization and pre-certification before outpatient procedures such as colonscopy and endoscopy can be performed. We will help to ensure that authorization is obtained however, we need your assistance. Please alert us to the requirement of your health plan. If you are uncertain as to the requirements, call the number listed on your benefit card.
Your Medical Record
Your medical record is confidential. We do not release information about your condition without your written permission. If you have specific advance directives, including Medical Directive or a Living Will, please provide us with a copy for inclusion in your medical record. Help us keep your medical record current by providing us with name and address changes, as well as changes in your telephone numbers, insurance carriers or referring physicians.

You may receive multiple bills for a procedure including, but not limited to: GI Associates of Big Bend professional charge, Capital City Surgical Center outpatient facility charge, and Capital Regional Medical Center outpatient facility charge. GI Associates does not process insurance billing for any outpatient procedures scheduled at Capital City Surgical Center or Capital Regional Medical Center. Hospitalization
Our physicians maintain privileges at Capital Regional Medical Center, and Select Specialty Hospital. We provide admission and consultation services at Capital Regional Medical Center. We perform outpatient endoscopic procedures at Capital Regional Medical Center.
During your hospitalization, your physician or a member of GI Associates of Big Bend, will visit you daily to assess your medical condition and to make any required changes in your hospital regimen. Prior to your discharge, you will receive specific instructions for your follow-up care. If you have questions and concerns, please ask us. You will receive a separate invoice from the hospital for all services provided to you during your hospital stay.
Medications
Please request prescription renewals during office visits, or call your pharmacist to determine if you have another refill remaining. We do not authorize prescription renewals in the evenings and on weekends when our on-call physician does not have direct access to your medical records. To assure our patient’s safety and well being, it is our policy to authorize prescription renewals only to patients who have an active record in our office, and have been seen within the past year.
To help us maintain continuity of care, be sure to bring all medications or a list of all medications that you are currently taking to each appointment.
